STA/Respite/MTA/SSRC

We know sometimes your carer may need to take a short break or you may need to have a break away from your usual residence. Our Respite and STA services are designed just for that. We also help participants transition from hospital/Aged care to community by offering Medium Term Accommodation. We are also registered to offer Specialist Substitute Residential Care (SSRC).

Community Nursing

Our Nursing team provides community based nursing care which covers Complex Wounds Management, Continence Assessments, Complex Bowel Care, Enteral Feeding and management, Urinary Catheter Management (In-dwelling, Suprapubic), Subcutaneous Injections, Day to Day Management of Medication. We work with our participants to develop a personalised care plan based on their needs and support requirements.

Assist Personal Activities

This is an individualized service (for both adults and children) where Connection Disability Services Workers assist you to complete daily activities that are relevant to your individual needs and support requirements.

Supported Independent Living

We now provide Supported Independent Living (SIL) for individuals that require higher supports for day-to-day living. We promote personal choice and strive to provide high quality support that is determined by the individual. Our holistic approach to SIL ensures that all aspects of a person’s life are nurtured to promote independence and wellbeing.
